Home Health Care is a form of health care service provided where a patient lives. Patients can receive home care services whether they live in their own homes, with or without family members, or in an independent or assisted living facility.
Home care delivery services provided by Medicare-certified agencies are tightly regulated. In order to receive health care services, the patient must be considered home bound. The definition of Home Bound per Medicare is, it must be a taxing effort for the patient to leave his or her home. This means they can go to the beauty shop, run errands, go to friends houses, but they need assistance getting to their destination, they need a walker, or when they return home they are physically drained. We want to work ourselves out of a job! It is not our goal to simply continue to care for our patients, but to see them return to health and be able to act indepentently of home health care. The purpose of home health is to promote, maintain, or restore a patient's health and reduce the effects of disease or disability.
Diabetes Management is where Heritage Home Health shines. We will visit the patient up to 4 times a day at their scheduled insulin times if needed. The goal is to manage the diabetes and educate the patient so they will be able to reduce the amount of insulin needed to control their diabetes.
YES! We work closely with your doctor and provide a service based on your doctor's orders. Many times, as conditions change, we are the eyes and ears of your doctor and will make suggestions toward your recovery. We are very successful with our communications and become a valued asset to your doctor.
We have access to skilled care 24 hours a day....every day! Our 24 hour a day access to skilled medical personnel with all the patient's current medical data is a great benefit to the patient and physician.
Studies show that over 70% of the elderly do not take their medication properly and/or take medications that have contradictory reactions. Heritage can view all the medications (even over-the-counter) and communicate their findings to the physician as well as monitor the medications to make sure they are taken correctly. Heritage also educates the patient on proper diet and side effects of their medications.

Physical Therapy - The purpose of physical therapy is to aid in recovery after surgery, counteract general weakness, lower the chances of falls, increase balance, and increase mobility.
Speech Therapy - Being able to communicate is vital and speech therapy will help those that were affect by stroke, illness, or accident. Speech therapy may also help those with swallowing problems that affect their ability to obtain proper nutrition.
Occupational Therapy- When people have strokes, sustain injuries, or have chronic mental or physical health problems, occupational therapists help them engage in occupations or everyday activities that are personally meaningful, socially satisfying, and culturally relevant. The goal is to help the patient accomplish their daily living tasks, learn new skills, adapt to permanent losses, fulfill their life roles, and participate fully in life.
